Practice Overview & Policy Agreement

Vital Mental Health Services LLC is committed to providing transparent, ethical, and patient-centered psychiatric care. These policies are designed to promote clear communication, continuity of care, and mutual understanding between patients and providers.

By receiving services from Vital Mental Health Services LLC, you acknowledge and agree to the policies outlined below.

If you are experiencing a medical or psychiatric emergency, call 911 or go to the nearest emergency room immediately.

General Practice Policies

1 Patient Financial Responsibility

Patients are responsible for payment of all services rendered, including evaluation fees, follow-up appointments, subscription fees, and any additional services provided. A valid credit or debit card must be maintained on file at all times.

2 Payment Methods

We accept the following payment methods:

  • Credit cards
  • Debit cards

3 No-Show and Late Cancellation Policy

To ensure appointment availability for all patients, we require at least 24 hours' notice for appointment cancellations or rescheduling.

  • The first no-show or late cancellation within a calendar month will incur an $80 fee.
  • Any additional no-shows ($140 rate) or late cancellations ($80 rate) within the same month will be charged at the standard appointment rate.
  • All fees will be automatically charged to the card on file.
  • Repeated missed appointments may result in discharge from the practice.

4 Telehealth Requirements

Most appointments are conducted via telehealth unless an in-person evaluation is clinically necessary. Patients must be physically located within Massachusetts at the time of their telehealth appointment in accordance with state licensing regulations.

5 Medication and Treatment Policy

Medications are prescribed during scheduled appointments. Prescription refills require active participation in treatment and adherence to the discussed plan of care.

  • Refills may be issued monthly or every 90 days depending on the medication prescribed and clinical appropriateness.
  • Controlled substance prescriptions require:
    • At least one in-person visit annually.
    • Ongoing monitoring in accordance with Massachusetts law and DEA regulations.

Failure to comply with treatment recommendations, monitoring requirements, or follow-up appointments may result in discontinuation of prescribing services.

6 Discharge Policy

Patients may be discharged from the practice for:

  • Repeated no-shows or late cancellations
  • Non-adherence to treatment recommendations
  • Failure to comply with medication management policies
  • Outstanding balances unpaid for more than 30 days
  • Disruptive, abusive, or inappropriate behavior toward staff

7 Emergency Policy

Vital Mental Health Services LLC does NOT provide emergency psychiatric services. If you are experiencing a medical or psychiatric emergency:

  • Call 911
  • Contact 988 for mental health crisis support
  • Go to the nearest emergency room

Do not rely on portal messages, voicemail, email, or text messages for urgent concerns.

8 Changes to Policies

Vital Mental Health Services LLC reserves the right to modify these policies at any time. Patients will be notified of significant policy changes.

Cash-Pay and Subscription Services

9 Initial Psychiatric Evaluation

The initial psychiatric evaluation includes:

  • Up to 45 minutes with a psychiatric provider
  • Comprehensive diagnostic assessment
  • Review of psychiatric and medical history
  • Treatment planning
  • Medication evaluation, as clinically indicated

Fee: $160

10 Monthly Subscription

The Monthly Subscription is designed for patients who require ongoing medication management and routine follow-up care.

Subscription Fee

$80 / month

Automatically charged to the card on file each month.

Included Services

  • Medication management visit every 3 months (and monthly if needed, up to 25 minutes)
  • Prescription refills as clinically appropriate
  • Secure patient portal communication regarding treatment questions
  • Ongoing psychiatric medication management

Additional Visits: If more than 1 additional appointment is required during the same month, they will be billed separately at the cash-pay rates.

11 Subscription Cancellation

  • Subscription fees are non-refundable once charged.
  • Subscription cancellation may affect prescription refill eligibility and continuity of care.
  • Patients must notify the office 1 week before their next billing date to avoid charges for the upcoming month.
